Insurance Information
Each of us is a participating provider in almost all of the
HMO's and
PPO's in the area. In particular, we do participate in all of the Blue
Cross/Blue Shield plans, Harvard Pilgrim Health Plan, Tufts Health
Plan, and Fallon Health Plan.
If your plan requires a co-pay for office visits, we do ask
that you pay it at the time of each visit. Payments can be made in the
form of cash, check, Master Card, Visa, or American Express. If you have
an indemnity
insurance plan, we will submit claims directly
to your insurance company as a courtesy to you. Please be aware that
the bill is
ultimately your responsibility, and that if there is a problem with the
insurance
company we will expect you to pay the bill and deal with the company
yourself.
We will only bill you directly if the insurer has not paid us within 45
days.
We do ask that you pay us at the time of the visit if you know that you
have
not met your insurance deductible. Please contact our office in regards
to any insurance questions or payment plan options.
If your plan requires referrals
for visits to specialists, it is your responsibility to obtain the
referral from our office staff well in advance of the visit, and to be
aware in advance of when referrals run out and need to be renewed. We
can only process a referral once you have made an appointment and have
an appointment date. Do not assume that we have taken care of the
referral if your doctor recommends that you see a specialist. You must
make an appointment first, then contact our office to request the
referral. Please allow our staff plenty of notice when requesting
referrals.
